2023 Flu Voucher Terms and Conditions

About your vaccination

  • You are required to book into the pharmacy where you will receive your flu vaccine.
  • You will be required to stay in the Pharmacy for 15 minutes after receiving your vaccination.
  • After the vaccine is given, the pharmacy is responsible for adding the details of your vaccination into the Australian Immunisation Register (AIR).

About your voucher

  • Please print out your voucher or have an electronic version including your unique voucher code and bring with you to your Pharmacy appointment.
  • You are required to have photo identification so the Pharmacist can verify your voucher code and administer your vaccination.
  • Customers who present without a voucher code will be charged the full price of the service.
  • The voucher code is for the use of the listed employee of the listed organisation on this email.
  • Each voucher has a unique code and can only be used once.

Payment and terms

  • Your flu vaccination voucher has an expiry date and cannot be used after the date on the voucher.
  • Once the vouchers have been issued to you, they are deemed as purchased by your company and you will be invoiced on the next billing cycle.
  • They cannot be returned and are not refundable or redeemable for cash.

Confidentiality

  • Harbour Healthcare will ensure any personal information you provide will remain confidential and private.
  • If your employer is paying for this service, your name may be disclosed for the purposes of verification, payment and invoicing only.
  • Personal information about you will not be identified in any reports that are generated as it will only include de-identified, aggregated health data and will be used for evaluating the service.
